LPN

The LPN provides patient care for the Cook Family Health Center, the primary care medical practice operated for Cook Group company employees and their families.

• Patient Intake & Assisting Provider
• CLIA waived testing
• Phlebotomy
• Scheduling
• Patient Education & Communication
• Administer medication/Injections, IV therapy
• Allergy Immunotherapy
• Nebulizer treatments
• Occupational nursing
• Electrocardiogram
• Sterile technique
• Immunizations
• Electronic Health Records data entry

• Must work and interact effectively with physicians and patients to insure quality health care

• Ability to remain calm and function in a stressful or emergent situation

• Must strictly adhere to Cook Clinic Employee Manual, Cook Code of Conduct, Cook Electronic Information Policy, HIPAA & OSHA regulations

• High School Diploma or equivalent required
• Valid LPN license in State of Indiana
• 2+ years nursing experience 
• Previous multi-line phone & customer service experience required
• Microsoft Office suite (Word, Excel, Outlook) experience required

Physical Requirements:

• Ability to walk & stand for long periods of time
• Able to bend, stoop, kneel or crouch
• Utilizes close visual acuity for working with computers, equipment, etc.
• Able to lift 20+ pounds occasionally
